NAME CHANGE APPLICATION
For an adult name change application, you must be 16 years of age or older and you must have lived in Ontario for the past 12 months. If you are 16 or 17 years of age you will need consent from every person who has legal custody of you. If you are married then you must give your partner or spouse notice that you are changing your name.
In Ontario individuals do not need to have a particular reason to have their name changed. Consideration should be given to what name you would like to use as after you receive the certificate of the name change, you will need to update most of your identity documents.
The processing time with the Government of Ontario is 6 to 8 weeks as long as the applications are properly completed and shown to be accurate. This does not include the processing time to change your government-issued documents, such as you passport, driver's license, health card etc.
A successful applicant will be issued a new birth certificate if born in Ontario and a change of name certificate with their new legal name(s).

Current Government Fees for Name Changes in Ontario:

Individual name change $137

For each child 17 years of age or younger is $22, if the application is submitted at the same time as the person who has legal custody of the child
When your application is completed and you receive your name change certificate, you can visit Service Ontario to have the following updated:

Health card
-
Ontario photo card

Vehicle registration

Ontario outdoors card
Your name change will be published in the Ontario Gazette. If publication can lead to significant harm, you can request that the name change be kept confidential.
